Our compassionate team will ensure throughout the referral and assessment process that you are comfortable, we can adapt methods for meeting and gaining information, meet you in your environment and ensure that people who are important to you have opportunity to be involved.

We will arrange an assessment within 48 hours of receiving a referral. 

Eligibility

  • Adults aged between 18 and 65 years and would like support in their own home. 
  • Individuals with low to complex mental health disorders, including but not limited to Attachment Disorder, Anxiety,Atypical Depression, Obsessive Compulsive Disorder (OCD), Bipolar Disorder, Personality Disorder, Depression, Schizophrenia.
  • Individuals currently residing in a time-limited service for people with mental health support needs.
  • Individuals ready for discharge from a mental health hospital.
  • Individuals who are entitled to Section 117 after-care under the Care Programme Approach (CPA).
  • A history of long-term hospitalisation, multiple admissions to the hospital or, at risk of being hospitalised.
  • Individuals who or may not need personal care assistance, this includes washing and dressing.
  • Individuals who need support with medication, shopping, accessing work, education and actives safely and companionship.
  • Individuals who have forensic history.
  • Individuals who are vulnerable to abuse, exploitation from others, or self-neglect.
  • We can support you to create a package of support that can meet your needs and be as independent as possible.

Referrals


Referrals can be made by a social worker, hospitals, courts, family members, GP’s etc. The assessment looks at the whole person to determine the accommodation is suitable for them and they will be able to achieve the most positive outcomes. Please contact us for further information.

Transition Plan

We encourage the individual and family to visit the accommodation once the assessment has been completed so they can meet our compassionate team and see their home for themselves. When the placement is agreed, the start date is agreed and a transition plan is created, to ensure that the transition is as smooth as possible. Each transition plan is based on individual needs and choice.


You will be allocated a key worker who will support you with your day to day living tasks. The key worker will take time to get to know your personality, and care needs. They will also be supporting the transition to ensure that it is successful.
